• Patient must be a Florida resident or seasonal resident
• Patient must be diagnosed with a qualifying condition.
• Review and evaluation of medical history and records by a qualified physician.
• Register qualifying patient in the Medical Marijuana Use Registry.
• The initial registration fee DOES NOT include the registration fee required by the Florida Dept. of Health.
