I have chronic pain and have a consultation with one of your physicians next week. What kind of records and documents will I need?

During this initial consultation you will need the following:

  1. Valid Florida Driver’s License or Florida Picture I.D.
  2. Recent MRI’s or CT Scans or X-Rays
  3. Any other pertinent information such as previous doctors notes or any other documentation.
  4. Must be at least 25 years old or older.
How long will this initial consultation take?

This initial consultation will take approximately 15 minutes, so that we can evaluate your paper work and the physician can determine if we can schedule you for a future appointment, or if there is additional paperwork needed.

Do you take insurance?

No, we are currently not accepting any types of Insurance but offer affordable self pay rates for all services.

I have been prescribed medication for my chronic pain. Should I take the medication only when the pain is unbearable?

No! It is always important to follow the directions just as the doctor has prescribed and take your medication regularly, even around the clock. You should not wait until the pain becomes severe to take the your medication, as pain is easier to control when it is mild.

IT IS DIFFICULT FOR YOU AND YOUR DOCTOR TO ASSESS THE EFFECTIVENESS OF YOUR MEDICATION IF IT IS NOT TAKEN AS PRESCRIBED.

If you find that your medication is not adequately alleviating your pain or you are experiencing unusual side effects,
contact our office immediately. You are required to bring all medications prescribed to you by the center
to any Medication Adjustment appointment.

What causes Chronic Pain?

There are a number of factors and conditions that can cause chronic pain. Chronic Pain can be caused by injury, accident, degenerative conditions, or other physical ailments.

Are there other treatments for chronic pain in addition to medications?

Yes! There is a treatment that can help to alleviate these painful conditions called Peripheral Nerve Stimulation (PNS).
While Peripheral Nerve Stimulation (PNS) is not a cure for chronic pain, it does help relieve these conditions. When used along with medications, this combination of treatment protocol allows the patient to achieve the proper relief from pain and thereby reducing the amount of medications required, and achieve the necessary pain relief.

Where can I acquire this type of treatment?
